First Visit

It really is all about you! From your very first visit, we want you to feel as comfortable and at ease as possible.

What to bring

  • Valid Drivers License or ID card and a second form of ID
  • A signed insurance claim form and your insurance card(s), if you have insurance
  • Full payment for services rendered (we have financing available)

 

What to Expect

When you arrive in the office you will be greeted by our friendly reception team. We will help you check-in, collect and verify insurance information, complete essential paperwork (forms will be sent electronically via text or email prior to your arrival to be filled out), and answer any questions you may have.

 

The first non-emergency visit will include:

  • A complete head, neck, and mouth examination
  • Any necessary X-rays
  • Complete charting of all existing dental work
  • Custom treatment plan (including costs)
  • Fluoride treatment (for children)
  • Set up any future appointments needed (including six-month cleanings)

When you’re finished with your appointment, the clinical team will walk with you to the front office where you will check-out with our friendly and helpful staff, they will assist you in scheduling your next appointment.  We understand sometimes the “unexpected” happens, therefore we have partnered with Care Credit and Wells Fargo to help ease any financial burden. We strive to provide the best customer care and find it very important to provide you with the best solution for the “unexpected”.
